We are committed to providing competent and humane services that respect individuals’ rights, protect their dignity, and promote their self-worth. Family Service Agency exists to strengthen people and communities through hope, healing, empowerment, and engagement. FSA strives to provide clients with a safe environment, regardless of culture or religious beliefs. Clients of FSA have the right to fair and equitable treatment, including, but not limited to, the right to receive services in a non-discriminatory manner and to the consistent enforcement of program rules and expectations. To maintain FSA’s longstanding reputation in the community, this Client Rights and Responsibilities Statement has been established to specify expectations.

Center for Counseling First Visit

Welcome! Upon calling to schedule your first appointment, please be prepared to provide (at minimum):

  • Name/s
  • Date/s of birth
  • Reason for seeking counseling
  • Insurance information

Please bring a photo ID, copay and/or insurance card (if applicable),and intake paperwork to your appointment.

Intake paperwork can either be completed on site, by arriving 15 minutes prior to the appointment time, or by printing off the documents.

We will attempt to meet your needs, to the best of our ability. If we are unable to provide the services that you need, we will suggest referrals to you.

No insurance?

If you do not have insurance, financial assistance is available and will be discussed with you.

Community Action First Visit

Welcome! After being assigned to your Family Support Specialist, please be prepared to provide (at minimum):

  • Name/s of all individuals in the home
  • Government issued identification (driver's license, birth certificate, etc.)
  • Proof of DeKalb County residency
  • 30 days of income

Your Family Support Specialist will provide a list of required documents before scheduling your first appointment.

We will attempt to meet your needs to the best of our ability. If we are unable to provide the services that you need, we will suggest referrals to you.

No social security card?

Community Action is dedicated to serving individuals regardless of their citizenship status. If you do not have a social security card, don't worry!
